Surgeon is a free but higher-level role. Sources disagree on the exact unlock level: the current Jobs reference lists level 60, an in-game role-select menu captured in a 2026 walkthrough showed level 40, and an older wiki infobox shows level 20. Marizma Games adjusts unlock levels between updates, so expect this number to keep moving — check the in-game role menu for the exact current requirement.

The Surgery wing sits on the 2nd floor: Scrub-In, three Operation Rooms, a Supply Room, and a Viewing Floor where other players can watch. Pre-Op is also on the 2nd floor, and patients are transported there once a Doctor's diagnosis calls for surgery (for example, Appendicitis cases move from the ER to Pre-Op after a consent form is filed).

Forceps, Betadine Bowl, Needle Driver, Laryngoscope, Lap Sponge, Sterile Saline, Betadine Swab, Kelly Clamp, Betadine, Sterile Gauze, Sponge Forceps, Saline Flush, and Sedative make up the standard operating table tray. A separate Crash Cart (Ambu Bag, Laryngoscope, Beta Blocker, IV Saline Bag, Prepared Epi, Saline Flush, Sedative, Adenosine, IV Lidocaine Bag) is used for cardiac emergencies like a Code Blue.
