Real Multi-Department Workflow
A Doctor orders a test, and a Lab Tech receives the sample through the pneumatic tube system to run it in the Laboratory. Results come back before treatment can continue, mirroring how an actual hospital shift moves between departments. It's a real dependency chain, not a single copy-paste interaction.
10 Roles With Specialization Ladders
Nurse and Patient are open at any level. Medic, Doctor, Veterinarian, Lab Tech, Surgeon, and Pharmacist unlock as you level up, and most have their own specialization tree on top of that. Nurse alone has 10 tiers running all the way up to Charge Nurse at level 360.
Emergency Color Code System
A Hospital Director can broadcast BLUE, RED, WHITE, YELLOW, BROWN, or PURPLE to the whole server. Each color is tied to a specific emergency, like Cardiac Arrest or a Violent Patient, giving every staff member a shared signal to react to in character. It's the game's way of turning a quiet shift into an all-hands event.

A basement plus three floors plus a rooftop helipad house dedicated ICU, PACU, Radiology, Labor & Delivery, and Security HQ departments. Getting from the ER to Surgery or from Reception to the Vet Clinic actually takes navigating a real layout. It's not just a couple of cosmetic ER rooms bolted onto a lobby.
Fully Customizable Patient Statuses
Patients pick from dozens of statuses, from treatable conditions like Flu and Broken Bone to purely cosmetic ones like Pregnant or Blind. Each status comes with its own customization options, from cast color to bump size. Staff then roleplay around whatever the Patient picked for that session.
Vet Clinic for Animal Roleplay
Added in March 2026, the Vet Clinic is a separate building next to the main hospital. A Veterinarian treats animal-exclusive statuses there, like Fleas and Rabies, that don't apply to human patients. It gives animal roleplay its own dedicated space instead of folding it into the main hospital wards.
The whole game loop connects at the Patient: they pick a status from the menu, and staff route them through the same real-hospital chain — vitals, a Doctor's diagnosis, a Lab Tech's test results delivered by tube, then either discharge or a transfer to Surgery. A Hospital Director watches over the whole server, ready to broadcast an Emergency Color Code the moment a Cardiac Arrest, fire, or violent patient breaks the routine, pulling Security or a Surgeon into the scene. It's this chain — department to department, role to role — that turns a simple status pick into a full shift.
